This gentleman presented with bulging lower eyelid fat ("baggy eyelids") and skin excess of the upper eyelid.
The incision of a transconjunctival blepharoplasty is performed at the inner side of the lower eyelids. The bulging fat is partially advanced over the bony inferior orbital margin. This diminishes the eyelid bulding and partially fills the retraction of the soft tissues at the level of the margin. For this high precision surgery through a small incision, general anaesthesia is preferable. The swelling can be considerable for several weeks, but there is no external scar.
In the same operative time, the skin excess of the upper eyelids was removed.
